Transitioning to Care - Dementia Support Australia

Dementia Support Australia (DSA), an initiative led by HammondCare and funded by the Australian Government, provides vital support for people living with dementia. They partnered with Punchy Studio to address a specific communication challenge, the stressful transition of patients from hospital settings into residential aged care. The goal was to provide aged care staff with practical, actionable strategies to reduce resident agitation and improve the settling-in process for both the individual and their families. We produced a character driven animated training video that follows the journey of Jack, a person living with dementia, and Harry, a care team member. The approach used relatable storytelling and soft, approachable visual design to illustrate five key steps for a successful transition. By utilising bespoke character animation and a clear, empathetic voiceover, the video simplifies complex care concepts into easy to follow instructions. This asset was deployed as a professional development resource for aged care providers across Australia, supporting the use of tools like the All About Me resource. The project involved comprehensive scripting, storyboard development, and motion graphics to ensure the educational content remained engaging and supportive for time poor healthcare professionals.
