Transitioning to care from Hospital - DSA (HammondCare)
Animated Explainer Video
Dementia Support Australia, a service delivered by HammondCare and funded by the Australian Government, helps families and care providers manage the behaviours and psychological symptoms of dementia. They came to Punchy needing to explain a specific and stressful moment in the care journey: the transition from hospital back into a residential or home care setting. Families often feel under-informed at this point, and the brief was to give carers a calm, practical resource that reduced anxiety and clarified next steps. Punchy produced an animated explainer that walked viewers through what to expect during and after hospital discharge, including communication between care teams, environmental adjustments, and signs that may indicate the person living with dementia needs additional support. The tone was warm and unhurried, with a character-led animation style and a measured voiceover. Services covered scripting, storyboarding, illustration, animation, voiceover and audio mix. The video sits alongside other DSA explainers in a broader carer education library.
